About The Position

Palliative Care Nurse Practitioner (NP) functions independently as an advanced practice provider, coordinating and assessing patient care needs, developing and modifying plans of care, and collaborating with palliative care interdisciplinary team. The NP completes regulatory requirements such as provider Intake visits, follow up, recertification and transition of care to hospice. Under physician orders or physician‑approved protocols, the NP may prescribe and adjust medications, including opioids, and order other therapies as patient condition requires. Additional responsibilities providing education to patients, families, and staff, and contributing to quality improvement initiatives.

Requirements

• Master's Degree Nursing
• At least two years of experience in hospice, acute care, or transitional care settings.
• Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
• Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
• Basic computer skills
• Ability to articulate and embrace integrated healthcare at home philosophy.
• Knowledge of hospice regulations, certification and recertification requirements, and Joint Commission standards.
• Advanced skills in pain and symptom management, end‑of-life care, and disease progression assessment.
• Strong counseling, collaboration, and interpersonal skills with patients, families, and care teams.
• Effective verbal and written communication skills in English.
• Strong organizational and planning skills, with the ability to prioritize tasks and meet deadlines.
• Ability to work independently while maintaining collaborative relationships with interdisciplinary teams.
• Proficient with electronic medical records and basic computer systems.
• Ability to function effectively in crisis situations with sound critical thinking skills.
• Active state‑issued NP license and RN license.
• California Nurse Practitioner License and California Nurse Practitioner Furnisher License, plus a California Registered Nurse License (all required upon hire).
• National Provider BLS – American Heart Association required upon hire.

• Doctor of Nursing Practice (DNP)
• Experience in advanced pain and symptom management, end‑of-life care, and hospice recertification assessments.
• Certified Hospice and Palliative Care Certification (CHPLN).

Responsibilities

• Comply with organizational policies, regulatory requirements, and safety standards.
• Conduct comprehensive medical, functional, and psychosocial assessments under physician supervision; perform and interpret diagnostic tests and procedures as defined by protocol.
• Independently prescribe, adjust, and discontinue medications (including opioids) per standardized procedures or physician orders.
• Perform Intake, follow up, and recertification visits.
• Develop and revise plans of care in collaboration with the interdisciplinary group (IDG); recommend treatment courses and monitor care.
• Provide education and counseling to patients and families regarding diagnoses, prognosis, goals of care, and treatment options.
• Assist families in decision‑making related to interventions, resuscitation status, and end‑of-life planning.
• Order medications including antibiotics and opioids, and manage standardized procedures or physician orders.
• Participate in interdisciplinary team meetings and provide patient information to physicians and other providers.
• Serve as liaison with physicians, discharge planners, and other departments regarding patient care.
• Provide education to staff, nursing facility teams, and families regarding hospice and palliative care needs.
• Participate in the development and implementation of evidence‑based education and protocols.
• Maintain schedules, productivity standards, and accurate documentation in the electronic health record (EHR).
• Perform other duties as assigned.
