About The Position

The Palliative Care Nurse Practitioner (NP) for Home Visits plays a critical role in delivering compassionate, patient-centered care to individuals with serious, chronic, or life-limiting illnesses in their own homes. This part-time position, requiring availability three days per week, focuses on managing symptoms, coordinating care, and improving quality of life for patients and their families. The NP will collaborate closely with interdisciplinary teams, including physicians, social workers, and hospice providers, to develop and implement individualized care plans. By providing expert clinical assessment, pain and symptom management, and emotional support, the NP ensures that patients receive holistic care tailored to their unique needs. Ultimately, this role aims to enhance patient comfort, dignity, and autonomy while supporting families through complex healthcare decisions in the home setting.

Requirements

  • Current and unrestricted Nurse Practitioner license in the United States.
  • Certification as a Nurse Practitioner with a focus on Adult-Gerontology or Family Practice.
  • Minimum of 2 years clinical experience in palliative care, hospice, or related nursing specialty.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
  • Ability to travel to patient homes within the assigned geographic area.

Nice To Haves

  • Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification.
  • Experience working in home health or community-based care settings.
  • Familiarity with electronic health record (EHR) systems and telehealth technologies.
  • Strong background in pain and symptom management for complex chronic illnesses.
  • Training in communication skills specific to end-of-life care and advanced care planning.

Responsibilities

  • Conduct comprehensive patient assessments during home visits to evaluate physical, emotional, and psychosocial needs.
  • Develop, implement, and adjust individualized palliative care plans in collaboration with patients, families, and healthcare teams.
  • Manage symptoms such as pain, nausea, fatigue, and breathlessness using evidence-based interventions and pharmacologic treatments.
  • Provide education and counseling to patients and caregivers regarding disease progression, treatment options, and advance care planning.
  • Coordinate care with primary care providers, specialists, hospice services, and community resources to ensure seamless support.
  • Document all patient interactions, assessments, and care plans accurately and timely in the electronic health record system.
  • Respond promptly to patient and family concerns, providing crisis intervention and support as needed.
  • Participate in interdisciplinary team meetings to review patient status and update care strategies.
