Schedule: Monday- Friday 8:00am-5:00pm Position Summary: The Palliative Care Nurse Coordinator is responsible for coordinating the clinical components of the Community-Based Palliative Care Program at Bluegrass Palliative Care. This role provides clinical case management, ensures continuity of care across settings, supports interdisciplinary collaboration, and assists in the development and implementation of clinical protocols to promote high-quality, patient-centered palliative care services.

Requirements

  • Graduate of an accredited school of registered nursing.
  • Currently licensed as a Registered Nurse (RN) through the KY Board of Nursing.
  • Current BLS certification required.
  • Minimum of five (5) years of professional nursing experience required.
  • Hospice and/or palliative care experience required.
  • Demonstrated expertise in symptom management, goals-of-care discussions, advanced directives, and end-of-life care principles.
  • Strong clinical assessment skills and sound nursing judgment.
  • Knowledge of hospice and palliative care philosophy and best practices.
  • Ability to manage emotionally complex patient and family situations with professionalism and compassion.
  • Excellent written and verbal communication skills, including timely and accurate clinical documentation.
  • Strong organizational, planning, and time-management skills.
  • Ability to manage multiple priorities and projects simultaneously.
  • Ability to collect, interpret, and utilize clinical and operational data.
  • Self-directed with the ability to work independently with minimal supervision.
  • Demonstrated ability to collaborate effectively with physicians, advanced practice providers, and interdisciplinary team members.

Nice To Haves

  • Experience working within an interdisciplinary team environment preferred.
  • Experience coordinating clinical services and providing education and feedback to staff preferred.
