About The Position

The Palliative/Geriatric Care Nurse Case Manager will be a member of the interdisciplinary palliative care team, responsible for assessing and educating members with serious and life-threatening diseases that are referred to the Palliative Care program. Under the direction of the Palliative Care Physician, the Nurse Case Manager serves as a liaison for patients, families of patients, care givers, and referring physicians. This position plays a pivotal role in the provision of palliative care consultations, as well as providing assistance in the primary care of seriously ill patients and their families. This person will need recent clinical experience since they may also need to perform basic clinical nursing duties, i.e., starting iv’s, blood pressure checks, escorting patients, preparing patients for the physician exam, and giving injections. The work for this role can cover the ambulatory setting and inpatient palliative care team. Candidate will perform duties within the scope of practice allowed by the State of Georgia.

Requirements

  • Unrestricted current Georgia RN License
  • BSN
  • 1-2 years clinical nursing experience in an area such as hospice nursing care, oncology, or palliative care
  • Ability to work independently in a self-directed environment
  • Valid Georgia driver’s license
  • Knowledge of case management principles, professional practice standards
  • Strong written and oral communication skills required and functional knowledge of computers
  • Ability to respond to common inquiries or complaints from physicians, staff, and patients.
  • Strong customer service skills
  • Experience in program development and policy and procedure development
  • Well organized, detail oriented
  • Willing to work with adults who are at the end of life with complex medical and social needs

Nice To Haves

  • Master’s degree in nursing
  • ARNP
  • 3-5 years’ experience, preferably in palliative care, oncology and/or hospice care program
  • Knowledge of regulatory nursing requirements and nursing standards of care
  • Change management skills
  • Demonstrates use of clinical skills and expertise in the area of pain management, cancer treatments and psychosocial impact of disease of patients and families
  • Possess a functional knowledge of chronic illnesses and as well as palliative care treatment.
  • Demonstrated abilities in establishing positive relationships with patients, families, hospital and clinic staff, and excellent case management, collaborative practice, organizational, communication and teaching skills
  • Experience in the development, coordination, and performance of educational and/or support programs
  • CHPN certificate

Responsibilities

  • Responsible for the day-to-day palliative care case activities and assigned caseload of patients.
  • Coordinates discharge planning with IPCC/team social worker as needed to the next level of care.
  • Communicates regularly with palliative care patients and family members to ensure follow through and adherence to treatment regime is successful.
  • Provides assistance with scheduling, coordination of treatments, requests for information and psychosocial support.
  • Serves as an advocate for patients and family members regarding their needs and concerns about their diagnosis, medications, and treatment options.
  • Communicates with all members of the healthcare team as appropriate about patient/family needs and concerns.
  • Provides direction to patients with unmet needs or gaps in resources, i.e., insurance, transportation, childcare, financial resources, language barriers, etc.
  • Facilitates access to appropriate community resources to address patient concerns so they may focus on treatment and recovery.
  • Refers to other team members as appropriate i.e., social worker, chaplain, physician.
  • Assists in the role as outpatient palliative care clinic nurse.
  • Duties include rooming patients and performing initial assessments before the physician’s exam.
  • Participates in IDT consultations and assists inpatient as the palliative care nurse in consultations.
