About the Role: This position primarily focuses on outpatient and home-based palliative care, with occasional inpatient consults to ensure seamless, continuous support. You’ll work alongside a collaborative team of physicians and advanced practice providers committed to compassionate, patient-centered care.
Requirements
- Board certified or eligible in Hospice & Palliative Medicine
- Passionate about community-based, patient-focused care
- Skilled at building trusting relationships with patients, families, and interdisciplinary teams
- Motivated to grow professionally within a collaborative, innovative healthcare system
Responsibilities
- Provide expert palliative and hospice care across home, clinic, and community settings
- Collaborate closely with a dedicated multidisciplinary team
- Participate in call and weekend rotations to support patient needs
- Offer continuity, comfort, and guidance to patients and their families
- Contribute to the growth and development of a service transforming lives
