About The Position

Under general supervision functions as a consultant, educator, and expert nursing or social work practitioner. Supports Senior Advisor and is accountable for statewide education and outreach for healthcare providers and patients/caregivers. Serves as organizational resource to ensure evidence-based practice.

Requirements

  • Bachelor’s in Nursing or Social Work required.
  • At least two (2) years’ experience as an RN or as a Social Worker in relevant clinical area.
  • Prior project management experience.
  • Current Maryland Registered Nurse licensure or Social Work licensure.

Nice To Haves

  • Master’s in Nursing or Social Work preferred.
  • CHPN or CHP-SW preferred.
  • Advanced knowledge of palliative care theory, practice and education.
  • Exceptional communication skills and ability to collaborate with multiple departments.

Responsibilities

  • Teaches and coaches healthcare providers in best practices for excellent patient and family care.
  • Acts as a best practice specialist on nursing or social work skills for the seriously ill, palliative, geriatric, and hospice patients.
  • Develops content and supports the Senior Advisor in the statewide palliative care education and outreach initiative.
  • Identifies ongoing provider and patient/caregiver educational needs through assessment of practice during ad-hoc field visits.
  • Participates in formal and informal education programs in area of expertise throughout the organization.
  • Collaborates with multiple leaders in a variety of departments across the organization.
  • Interprets and implements research findings into the clinical practice setting as appropriate.
  • Leads, facilitates, and/or participates in Process Improvement Teams.
  • Serves as a resource for clinical policy and procedure development and dissemination.
