Requirements

  • 3-5 years of clinical experience or a comparable combination of education and experience required.
  • Experience with Palliative care and/or Hospice programs is highly desirable.
  • Registered Nurse (RN) license required.
  • Ability to relate and maintain a rapport with all levels of professional personnel.
  • Computer experience and knowledge in the management of computer data desirable.

Responsibilities

  • Developing, implementing, and evaluating palliative care and hospice services.
  • Enhancing the provision of high quality palliative care services to seriously ill and dying patients in the hospital setting.
  • Developing and maintaining strong relationships with physicians, hospital staff, community stakeholders, and system peers.
  • Participating in quality improvement activities.
  • Providing formal and informal education and support to staff, patients, and their families.
  • Consulting on complex cases and appropriateness of care or level of care/utilization activities.
  • Acting as a liaison to internal and external contacts/committees/programs.
  • Ensuring compliance with regulatory/accreditation agencies.
  • Facilitating program development and leadership.
  • Assisting with decision making/problem solving regarding hospice services.
