Collaborates with the Palliative Care Services team in the process of planning, coordinating and evaluating patient care activities to achieve department goals and maintain high quality service. Oversees and coordinates education for patients, families and staff. Samaritan Palliative Care supports patients with serious illness and those who care for them in living with hope, resilience, and dignity throughout their journey. With attention to education, support, and resource stewardship, we address physical, social, emotional, and spiritual wellbeing. Our community-based program offers these services in the home setting which may include the patient#s own residence, skilled nursing facilities, assisted living facilities and adult foster homes.
