About The Position

Big Bend Palliative Care (BBPC), formally known as Transitions Supportive Care (TSC), a branch of Big Bend Health and part of the 7 Oaks Health family, is a community-based palliative care program providing an extra layer of support to patients and families facing serious illness. Big Bend Palliative Care partners with providers and community organizations to deliver care that prioritizes comfort, clarity, and quality of life. We are looking to bring on a part time APRN 3 days a week. The ideal candidate for Big Bend Palliative Care's APRN position is a licensed Advanced Practice Registered Nurse with advanced clinical training who provides comprehensive palliative and supportive care services in accordance with the state Nurse Practice Act and under the supervision of a collaborating physician. They are skilled in conducting thorough patient evaluations, diagnosing and managing medical conditions, developing and directing individualized plans of care, and prescribing appropriate treatments. The ideal candidate will collaborate effectively with physicians, interdisciplinary team members, patients, and families, perform in-home, facility, or clinic-based visits, assess eligibility for supportive care services, and utilize community resources to coordinate care that meets the needs of individuals and families across the continuum of care.

Requirements

  • Master's degree or doctorate in Nursing from an accredited advanced practice school of nursing for nurse practitioner.
  • Licensed in the state of Florida as an Advanced Practice Registered Nurse (APRN).
  • Must have BLS/CPR certification.
  • A minimum of two (2) year Nurse Practitioner/ APRN experience.
  • Minimum of one (1) year of relevant clinical experience in hospice, palliative care, or a related specialty (e.g., primary care, geriatrics, oncology, cardiology, pulmonary, or complex chronic disease management.
  • Knowledge of computers systems, Microsoft office, along with good organizational skills.
  • Excellent observation, verbal and written communication skills, critical thinking skills, interpersonal skills, and mathematical skills; nursing skills per competency checklist.
  • Pediatrics: Must have experience and/or training in the developmental stages and needs of infants, children, and adolescents.
  • Demonstrates knowledge in communication and counseling patient/family in dealing with serious illness issues.
  • Valid driver’s license, auto insurance, and reliable transportation.
  • Active and Cleared APRN license

Nice To Haves

  • Certification in a specialist area preferred; (e.g., Hospice and Palliative Nursing (ACHPN), Pediatrics or Geriatrics).
  • Knowledge of palliative/supportive care principles and the end-of-life process;

Responsibilities

  • Provides comprehensive palliative and supportive medical care in accordance with the Nurse Practice Act, under physician collaboration, including assessment, diagnosis, treatment, prescribing, and referral management
  • Develops, implements, and coordinates individualized plans of care in collaboration with physicians, interdisciplinary team members, and community providers to ensure effective symptom management and continuity of care
  • Conducts patient visits across home, facility, and clinic settings; evaluates eligibility for supportive services; and supports hospice-related care needs, including face-to-face visits and on-call rotation
  • Educates patients, families, and caregivers on symptom management, treatment options, advance care planning, and psychosocial and spiritual needs, aligning care with patient and family goals
  • Completes accurate, timely clinical documentation; participates in interdisciplinary meetings; and serves as a clinical resource to team members and community partners to support program growth
  • Ensures compliance with Medicare, Medicaid, state and federal regulations, HIPAA, and conditions of participation; contributes to quality improvement initiatives and maintains professional competence through continuing education
  • Performs other duties as assigned
