As a designated regional referral center Trinity Regional Medical Center offers a wide range of primary and secondary care services. General Established team of 1 Medical Director and 4 Advanced Practitioners Established practice serving the community Experienced staff, both clinical and non-clinical, who know the patients and the community Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination

Requirements

Nurse Practitioner or Physician Assistant
Ability to work as a team member
Hold an unrestricted medical license in the state of Iowa
Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice

Nice To Haves

Experience in post-acute and/or long term care facility preferred
Palliative Care certification preferred

Responsibilities

Average of 8-10 patients per day; no nights or weekends
Shared call amongst the providers. Currently 1:17 – Triaged through MyNurse
Assume PCP role while rounding in facilities to meet nursing home requirements and acute needs of patients with a focus on patient driven goals of care and quality of life
Clinic hours: Monday-Friday 8am-5pm, no nights, weekends, or holidays, 4 day work week option available.
Evaluates, interprets, and coordinates information to make decisions and recommendations regarding patient care
Patient-focused, friendly, and willing to collaborate in a team environment
Excellent communication (written and verbal), analytical and problem-solving skills
