About The Position

The PACT (Placement & Control Team) Case Manager is responsible for all post-intake care delivery, including rapid placements, ongoing control visits, caregiver support, compliance management, and long-term relationship ownership with families. After FAST (First Assessment Specialist Team) hands off the approved case, PACT owns everything from placement through annual renewals, ensuring families receive stable, compassionate, and compliant long-term care. Case Managers operate with high-degrees of schedule autonomy, work within regional clusters, and alternate control visits with their RN counterparts. Pact clinical members are the backbone of long-term AFC success and build enduring, trust-based relationships with families.

Requirements

  • Bachelor’s degree (preferred in social work, human services, psychology, or related field)
  • 2+ years working with elders or adults with disabilities
  • Valid driver’s license + travel throughout Massachusetts and willingness to be on the road (typically within 60-90 minute radius of your home city)
  • Strong clinical assessment skills and comfort with independent decision-making
  • High empathy and communication skills for working with vulnerable adults and families
  • High attention to detail and documentation quality
  • Ability to use Microsoft Office, Teams, and electronic data portals
  • Comfort operating in a fast-paced, high-volume, rigorously measured environment

Nice To Haves

  • Master's degree in social work or a related field.
  • Experience working in Adult Foster Care or a similar setting.
  • Certifications in gerontology or case management.
  • Social worker license from the Massachusetts Board of Registration in Social Work

Responsibilities

  • Conduct placements within 24–48 hours of approval, jointly with the assigned RN.
  • Perform ongoing control visits (alternating between RN and CM per new MassHealth rules).
  • Maintain strong, trust-based relationships with families and ensure consistently “insanely great” support.
  • Develop, implement, and adjust individualized care plans tailored to client needs.
  • Support caregivers in meeting continuing education requirements.
  • Lead annual renewals, ensuring no lapse in coverage through proactive documentation collection and follow-up.
  • Identify family referrals and support Entyre’s growth within each regional cluster.
  • Maintain high-quality, accurate documentation in all case records.
  • Coordinate closely with PACT-Supervisors on caseload balancing, quality, and continuous improvement
