About The Position

The Program for All-inclusive Care of the Elderly (PACE) Transportation Coordinator/Driver on an as needed basis will assist the PACE Transportation Supervisor with various duties. This includes working as a member of the PACE Interdisciplinary Team (IDT) to assess participant transportation needs and make recommendations, determining appropriate modes of transportation services, and serving as the liaison with Four County Transit and local providers. The Coordinator must maintain effective communication with providers, drivers, and the medical community, work independently, and use sound judgment in coordinating transportation services in accordance with federal, state, and local guidelines.

Requirements

  • Associate degree or high school diploma with equivalent experience.
  • Minimum two (2) years of experience in transportation, human services, disabilities, or related field.
  • Advanced knowledge of computers.
  • Excellent interpersonal communication skills.
  • Valid driver license and ability to pass a criminal background check.

Responsibilities

  • Assist the PACE Transportation Supervisor in receiving referrals for potential PACE transportation services.
  • Conduct in-home assessments on potential PACE participants to determine individual needs and evaluate barriers to service delivery.
  • Serve as a required member of the PACE Interdisciplinary Team when the Transportation Supervisor is unavailable.
  • Attend morning meetings to report on occurrences related to transportation and PACE participants.
  • Pre-authorize all PACE transportation services in accordance with PACE guidelines.
  • Assist in training PACE Drivers and problem solve as issues arise.
  • Develop and maintain a local transportation provider network.
  • Schedule transportation through Four County Transit or the local provider network.
  • Schedule PACE Drivers and coordinate transportation services in a timely manner.
  • Communicate with service providers to determine satisfactory completion of trips and address any problems.
  • Coordinate the most cost-effective and efficient transport of PACE participants.
  • Establish and maintain good working relationships with PACE participants, caregivers, and the medical community.
  • Conduct ongoing evaluations of existing transportation provider services.
  • Ensure maintenance of PACE vehicles.
  • Utilize problem-solving skills for non-routine transportation requests.
  • Perform duties of PACE Driver when necessary.
  • Generate all required PACE program reports.
  • Comply with HIPAA requirements and maintain confidentiality.
  • Attend required training.
  • Perform other appropriate duties as assigned.
