About The Position

The PACE Home Care/Case Manager RN promotes and maintains participants’ health in the community through education, counseling, and coordination of preventive and rehabilitative services with the interdisciplinary team. The RN assesses health needs, oversees and implements nursing care plans, and provides direct nursing care. The role includes ongoing re-evaluation of individual and family needs. The Home Care/Case Manager RN coordinates scheduling, communication, and documentation to ensure efficient home care services. The RN also oversees and updates the CNA plan of care. What is PACE? PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 180 PACE programs, serving more than 80,700 participants in 33 states. The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.

Requirements

  • Graduated from an accredited school of nursing is required.
  • Current, valid Registered Nurse (RN) license in the state of Colorado, maintained throughout employment.
  • Valid Colorado driver’s license, reliable transportation, and proof of required automobile insurance maintained throughout employment.
  • Must possess and maintain current CPR certification throughout employment.
  • Ability to set priorities, manage competing demands, and consistently follow through.
  • Strong interpersonal, verbal, and written communication skills.
  • Ability to communicate clearly, effectively, and respectfully in a wide range of situations, including highly emotional or stressful interactions.
  • Proficiency with electronic health record (EHR) systems and Microsoft Office applications.

Nice To Haves

  • Bachelor’s degree in Nursing is preferred.
  • One to two years of recent hospital nursing or other acute setting; home health and/or LTC experience preferred.
