About The Position

Full-time physician available immediately to provide primary care services to frail participants of the Neighborhood PACE of NeighborHealth program, a nationally recognized service delivery model for community frail older adults. Neighborhood PACE is a founding member of the national PACE program (Program of All-Inclusive Care for the Elderly), now in operation for 35 years. Care in all settings is provided in concert with an interdisciplinary care teams of health professionals in a progressive health care organization. Candidate will provide geriatric primary care services for PACE participants across multiple long term care settings, including in the PACE clinics, at home , and in assisted living and nursing facilities. Strong preference for a candidate with significant experience as a practitioner for frail adults. Spanish proficiency preferred but not required. Candidates with certification in Internal Medicine or Family Medicine will be considered. Geriatrics certification desirable but not required. Competitive salary with excellent benefits.
