About The Position

Care Guide Partners PACE is a part of Hosparus Health. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age. PACE helps seniors who need nursing home-level care maintain their independence by providing all the medical care and support services they need to continue living comfortably and safely at home. Schedule: 24 hrs. per week Position Summary: Under the joint direction of the Care Guide Partners PACE Medical Director and Executive Director, the PACE Nurse Practitioner is responsible for assessing, initiating, providing and coordinating direct care of participants in the Care Guide Partners PACE (CGPP) program. Assigned participants are primarily seen in the clinic/day center but may also be in other settings such as a home, long term care facility, assisted living facility or hospital. The PACE Nurse Practitioner supports and coordinates care with the interdisciplinary team (IDT), as well as other medical providers, as needed and participates in education, outreach and leadership activities for the communities served. The PACE Nurse Practitioner may be assigned additional patient care or administrative duties at the discretion of the Medical Director and/or Executive Director.

Requirements

  • Licensure (or eligibility for licensure) for advanced nursing practice in KY, including DEA and controlled substance permits, required.
  • Minimum of one year experience in full time advance nursing practice required.
  • Knowledge of physical, mental, and social needs of frail older adults.
  • Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire.
  • Must maintain Basic Life Support certification.
  • Must have flexibility to travel within service area and provide infrequent care in participant homes.
  • Proficiency with technology-based tools, including electronic medical record.
  • Able to work independently and as an effective member of a team. High level of autonomy is required for clinical and management decisions.

Responsibilities
