The PACE APRN exemplifies and promotes advanced nursing practice. Provides direct participant care and evaluates participant outcomes. Sets and evaluates practice standards. Provides consultation to nurses. Maintains own professional knowledge and skills as an expert and role model in the PACE Center’s advanced nursing practice. Works in close collaboration with Bluegrass Care Navigator’s Chief Medical Officer and the PACE Medical Director as well as community referral sources and medical specialists to establish the individualized participant centered plan of care (POC). Assists in providing a plan of care for the participant/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, federal and state guidelines, and organizational policies and procedures, incorporating participant and caregiver input and requests as deemed appropriate. Providers of Bluegrass PACE care deliver the full range of ambulatory primary care services, including diagnosis and treatment of acute and chronic illness in geriatric age groups, preventative care, minor surgical procedures, and occasional medical emergencies. The providers are responsible for seeing their scheduled participants as well as walk-ins and add-ons and provide coverage during the operating hours of the Center. Provide telephone consultation as needed, perform administrative tasks associated with the provision of medical care, participate in quality improvement activities and provide community service. Medical providers are expected to manage all participants and conditions within the scope of their training and approved privileges.
