Job Summary Assess, determine and provide a full spectrum of psycho-social support programs to patients as needed. Responsible for keeping self and management up to date on all state/federal/local support programs.

Requirements

  • Master’s degree in psychology, social work or related field
  • Minimum two years’ experience
  • CPR Certification (including BLS Child, Infant, Adult with one-time hands-on skills training)
  • Ability to interface with all levels of personnel in a professional manner, including people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs
  • Ability and willingness to meet the organization’s attendance and dress code policies
  • Ability to handle confidential materials and information in a professional manner
  • Excellent customer service skills and commitment to providing the highest level of customer satisfaction
  • Experience/familiarity with Practice Management and Electronic Medical Records systems, NextGen preferred
  • Experience/familiarity with computers, business e-mail, communication systems and internet search capabilities; proficient in Microsoft Office products, specifically Word and Excel

Responsibilities

  • Evaluate psychological status of patients within four weeks from the date entering care
  • Study patient’s psychological status once per trimester
  • Develop individual care plan for each patient, based on results from the initial evaluation
  • Counsel patients considered high risk upon need and appropriate follow up
  • Conduct psycho-social group education including postpartum educational groups
  • Maintain current community resources file, follow up with patient and referral agencies
  • Consult with physicians, nutritionist, and coordinator on a weekly basis regarding high risk patients
  • Document and record functioning level of patient intervention contacts, progress/notes forms
  • Arrange appointment for patients who should attend post partum support groups
  • Complete postpartum assessment after patient delivers
