The PACE Enrollment Specialist plays a critical role in supporting the intake and enrollment process for the Program of All-Inclusive Care for the Elderly (PACE). This position is responsible for guiding prospective participants and their families through the eligibility, application, and enrollment process, ensuring compliance with federal and state regulations. The Intake and Enrollment Specialist is a liaison between the community, participants, and the interdisciplinary team to facilitate a smooth transition into the PACE program including visiting home and community settings.

Requirements

  • A minimum of 1 year of experience in healthcare, senior services, or enrollment coordination is required.
  • Additionally, 1 year of experience working with geriatric or frail populations is required.
  • Bachelor’s degree in Social Work, Healthcare Administration, Nursing, Public Health, or a related healthcare or business field, or equivalent relevant work experience is required.
  • CPR (BLS or ACLS) required.
  • Valid Oregon Driver’s License is required.
  • Strong attention to detail, ensuring accurate enrollment coordination.
  • Excellent organizational skills and the ability to manage multiple tasks effectively.
  • Exceptional interpersonal and communication skills, maintaining a friendly and approachable demeanor.
  • Ability to work collaboratively within a team environment.
  • Proficiency in using various software applications, including Word, Excel, PowerPoint, and Outlook.
  • Capability to work independently with minimal supervision.
  • Each member of the PACE organization's staff that has direct contact with participants must meet the following conditions:
  • Be legally authorized (for example, currently licensed, registered or certified if applicable) to practice in the State in which he or she performs the function or action.
  • Only act within the scope of his or her authority to practice.
  • Have 1 year of experience working with a frail or elderly population
  • Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.
  • Be medically cleared for communicable diseases before engaging in direct participant contact.

Responsibilities

  • Educate prospective participants, families, and caregivers about the PACE program, services, and eligibility requirements.
  • Meet with potential participants to guide them and their families through the enrollment process.
  • Conduct initial screenings and coordinate assessments to determine program eligibility.
  • Assist with the completion and submission of enrollment applications and required documentation.
  • Schedule and facilitate home visits, center tours, and interdisciplinary team evaluations.
  • Maintain accurate and timely records in compliance with HIPAA and regulatory standards.
  • Collaborate with marketing, outreach, and clinical teams to support enrollment goals.
  • Track and report enrollment metrics and trends to leadership.
  • Provide culturally competent and compassionate support to diverse populations.
  • Ensure compliance with CMS and state-specific PACE enrollment guidelines.
  • Provide administrative support as needed.
  • Collaborate on outreach initiatives to promote the center’s resources within the community.
  • Serve on various internal and external committees as required or designated.
  • Participate in team meetings, caregiver groups, and special events as needed.
  • Foster a safe and supportive environment conducive to the professional development of healthcare professionals.
  • Meet department and company performance and attendance expectations.
  • Follow privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
  • Perform other duties as assigned.
