About The Position

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care. JOB SUMMARY The Caregiver is responsible for providing personal services to assigned patients. Provide direct patient care, collaborate with the other Interdisciplinary Team (IDT) members within the care team, and with other health care providers in various care settings. Provide patient care in a manner that promotes an interdisciplinary team approach, respects patient/family goals for care, meets Colorado State Regulations and requirements and reflects the mission and philosophy of TRU Community Care. Upon request, participates in all areas of clinical services.

Requirements

  • Must possess a high school diploma or GED.
  • QMAP license required or must obtain within 90 days of hire.
  • CPR and 1st Aid certification required.
  • Ability to perform simple arithmetic calculations.
  • Knowledgeable of emergency procedures.
  • Ability to stay calm in emergency situations.
  • Excellent interpersonal skills to maintain professional and courteous relationships with patients, families, and visitors.
  • Strong verbal and written communication skills.
  • Professionalism and ability to be discreet with confidential and sensitive issues.
  • Must be detail orientated.
  • Must be able to work a flexible work schedule including weekends.
  • Must maintain effective working relationship with employees, patients, co-workers, and the public.
  • Ability to handle multiple tasks, projects, and meet deadlines.
  • Ability to arrive to work on time and according to schedule consistently.
  • Ability to use gait belt and sit to stand after orientation to devices.
  • Must have reliable transportation to and from work.
  • Must successfully pass agency required background checks and drug screening.
  • Must comply with agency health screening and immunization requirements.
  • Must be able to demonstrate computer competency.

Nice To Haves

  • 1 year of caregiving experience preferred, and additional CNA license preferred.

Responsibilities

  • Performs and records various duties necessary to provide complete care within established standards, policies, and procedures including but not limited to taking vital signs, providing patient hygiene, positioning patients, collecting specimens, determining intake, output, and mass, feeding patients (if allowed in the care setting, and once shown how to properly do so), making beds and delivering messages.
  • Assists in maintaining patient and unit area in a clean, orderly, and safe manner.
  • Assists in sorting and retrieving equipment and supplies.
  • Assists with the admission, transfer, and/or discharge of patients by transporting, lifting and position patients while following necessary safety precautions.
  • Assists with medical procedures as directed.
  • Participates in ongoing training as required.
  • Completes other tasks/duties as assigned.
  • Incorporates patient/family preferences and goals within the plan of care.
  • Supports and respects the patient’s and family’s unique diversity, including spiritual and cultural beliefs.
  • Completes documentation in an accurate and complete manner as per agency standard and in compliance with regulatory standards.
  • Provides patient and family education as appropriate; areas of education include, but are not limited to personal care, medications, procedures, safety, diet and nutrition, disease process, death and dying.
  • Contributes to the team, utilizes team resources, supports team decisions, and maintains positive working relationships with all team members.
  • Works collaboratively with volunteers and provides ongoing support and guidance.
  • Demonstrates effective verbal, written, and electronic communication skills; gives and receives feedback in a timely open manner; engages collaboratively in conflict resolution processes as needed.
  • Is flexible and responsive in meeting and exceeding the expectations of all customers, both internal and external
  • Participates in mandatory educational activities and required meetings.
  • Is competent using agency information and technology systems
