About The Position

Under the direction and supervision of the Home Care Supervisor, the PACE Escort is responsible for providing escort services to participants, and coordinating support between their homes, medical appointments, and the PACE center. This role also includes assisting participants in their homes and within the PACE center as needed, while maintaining a strong focus on safety, comfort, and dignity.

Requirements

  • Must complete and pass the North East Medical Services PACE training curriculum upon hire.
  • Current BLS and First Aid required
  • Ability to communicate effectively.
  • Must be able to fluently speak, read and write English.
  • Fluency in Chinese (Cantonese and/or Mandarin) is required.

Nice To Haves

  • Minimum one year of experience as a Personal Care Aide, Certified Nursing Assistant, or EMT.
  • Knowledge of the geriatric population.
  • Knowledge of safe and sanitary personal care techniques for elderly population with physical with physical limitations.
  • Knowledge of proper body mechanics during provisions of assistance with Activities of Daily Living.
  • Home Health Aide certification
  • Certified Nursing Assistant (CNA) certification
  • Minimum one year of experience working with a frail and/or older adult population
  • Fluency in other languages is an asset.

Responsibilities

  • Escort participants between their homes, medical appointments, and the PACE team in a safe and timely manner.
  • Accompany participants during transport to ensure comfort, safety, and adherence to care needs.
  • Assist participants with entering and exiting vehicles, including use of durable medical equipment (e.g. walkers, wheelchairs).
  • Provide basic non-medical support during escort and transport, including personal care, reminders, reassurance and observation.
  • Ensure participants are properly checked in and handed off to appropriate staff at medical facilities or the PACE center.
  • Maintain confidentiality and respect participant privacy at all times.
  • Provide in-home support in accordance with participant’s care plan and in coordination with the interdisciplinary team (IDT), participant and participant’s family.
  • Support participants within the PACE center, including supervision, task assistance, and emotional support.
  • Observe, track and report changes in participant condition, including vital signs, intake, and output, to the Home Care Supervisor or appropriate staff.
  • Communicate health and safety concerns promptly to the Home Care Supervisor and/or other program staff.
  • Follow safe work practices, including proper body mechanics and universal precautions.
  • Interact professionally and courteously with participants, families, and caregivers.
  • Participate in social and recreation activities with participants as appropriate.
  • Assist with covering assignments in other areas as needed for daily operations.
  • Utilize Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic.
  • Perform other job duties as required by manager/supervisor.
