About The Position

The position involves effectively communicating with participants, families, staff, and other agencies and departments. The primary responsibility includes driving a specially equipped van to deliver physically limited persons to varied destinations, ensuring that wheelchair lifts and tie-down equipment are properly operated, and that participants are delivered to appointments in a timely manner. The role requires following established schedules, adjusting them to meet unanticipated changes, and determining the safest and most efficient pick-up and delivery routes. Additionally, the employee assists participants on and off the van in a safe and courteous manner, deals with medical equipment and emergencies, inspects and maintains the vehicle, and ensures all necessary program records are maintained according to policy. The employee must observe each participant for any changes in their condition and report such changes to the licensed nurse, complete required documentation, and assist in the coordination of 24-hour care delivery with the FHCN Navigation Center. Other projects and duties may be assigned as needed.

Requirements

  • Must possess a high school diploma or General Educational Development (GED) certificate.
  • Must be at least 21 years old.
  • Minimum 1-year experience transporting individuals with special mobility needs.
  • Ability to interact in a friendly and professional manner with participants and their families.
  • Effective communication with other Interdisciplinary Team members on patient status and needs.
  • Must meet a standardized set of competencies for the specific position established by FHCN PACE.
  • Must be medically cleared for communicable diseases and have all immunizations up to date.
  • Must pass a criminal background check.

Nice To Haves

  • Ability to prepare basic correspondence and simple reports in Microsoft Word.
  • Ability to use Microsoft Excel to create tables and simple displays of information.
  • Ability to create basic presentations in Microsoft PowerPoint.

  • Assists in the coordination of 24-hour care delivery with assistance from the FHCN Navigation Center.
  • Performs other projects and duties as assigned.
