As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. PACE Driver 5p - 12a PACE Transportation Coordinator/Driver Position Summary: Under the supervision of the PACE Transportation Coordinator, the PACE Driver is responsible for the direct transportation of PACE participants to and from their destinations including, but not limited to, transportation to/from the center/day clinic and to/from appointments related to approved PACE covered services. The driver has continuous interaction with participants, caregivers/families and community agencies and plays an integral part in communicating changes in participants to the PACE Transportation Coordinator for input to the Interdisciplinary Team (IDT). Requirements

  • High school diploma or general education degree (GED) required.
  • 1 year experience transporting individuals with special mobility needs.
  • Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire.
  • Able to work positively in a diverse and stressful environment.
  • Must have, or willing to obtain, first aid training.
  • Must maintain CPR certification.
  • Must have a valid driver’s license in accordance with policy with proof of automobile insurance required.
  • Knowledge of safety and infection control requirements for a healthcare environment.
  • Knowledge of automobile and medical equipment use and maintenance.
  • Ability to communicate clearly and effectively.
