As a BCN Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Most care services are provided at the PACE Center, including transportation to and from the Center. Be on the forefront of holistic care for our Kentucky community by joining our PACE program team! The PACE Activities Coordinator plans, develops, implements, and oversees recreational activities for PACE program participants. The Activities Coordinator coordinates volunteer services in the PACE Center as participant needs are identified in collaboration with the Center Director, IDT members, and the Bluegrass Care Navigator Volunteer Department.
