Start Date: January Rate: $80/hr Interstitial Lung Disease Patient Population- The primary role is to provide support for urgent clinic visits (sick visits and post-hospital follow-ups) for patients established with the ILD group. The majority of the urgent visits involve 1) assessing severity of illness (treat as outpatient, referral to ED or need for admission) in conjunction with the patient's provider 2) providing guidance for those being treated as an outpatient (obtaining RVPs in clinic, prescribing oral antibiotics, or supportive measures) 3) in conjunction with the ILD nurse, determining if O2 needs have changed and adjusting prescriptions accordingly. For post-hospital follow-up visits, the APP will review medications from hospital discharge, assess patient/caregiver understanding of any medication changes, and confirm that the patient is stable/improving from the time of discharge. The APP will be in the clinic with an MD provider who will serve as a resource for the APP. Two half-day sessions/week are needed. The temporary APP would be oriented by our current APP prior to starting.

Requirements

  • Must be certified and licensed as a Nurse Practitioner or Physician Assistant in the State of Maryland.
  • Current CPR certification required
  • Must maintain current licensure and certification throughout employment.
  • Completion of the JHH credentialing process is required prior to the start date.
  • Excellent organizational skills and the ability to handle multiple tasks simultaneously.
  • Analytical ability is necessary to evaluate a patient's health status and implement a proper care plan. Interpersonal skills are necessary to communicate with other services.
  • Ability to work well as a member of a health care team with a high degree of self-motivation.
  • General computer knowledge and the ability to use computerized medical information systems.
  • Knowledge of research methodology and working knowledge of computers.
  • Must be able to independently assimilate and evaluate clinical data.
  • Must be willing to assist in writing protocols and protocol revisions and manuscripts preparation.

Responsibilities

  • Urgent sick and post-hospital follow-up visits for Interstitial Lung Disease patients.
  • Assess the severity of illness, provide guidance for outpatients, and adjust O2 and prescriptions as needed.
  • Monitor and follow up on diagnostic test results, prescriptions, medical equipment, insurance requirements, and documentation needs for patients for the 500+ patients in the ILD program
  • Telephone triage for patient problems and response to therapy, and communicate with the program physicians.
  • Educate patients on the ILD inpatient and outpatient service about the disease process and current and investigational therapies.
  • Educate and train hospital nurses and house staff about the care of patients with advanced ILD.
  • Round on inpatient service with the Pulmonary consult service when needed.
  • Screen medical records and facilitate scheduling of new referrals to the ILD Clinic.
  • Facilitate and obtain insurance authorization for medical therapy, diagnostic testing, and hospital admission.
  • Coordinate and develop relationships with industry representatives.
  • Coordination and Organization of the Johns Hopkins Pulmonary ILD Patient Support Group.
  • See patients in the outpatient clinic for follow-up or for urgent visits.
  • Responsible for supervising the research activities for the ILD Program
  • Assist with coordination of industry and internal ILD clinical trials, including identifying subjects, IRB submission, regulatory documentation and protocol required study visits and testing.
  • Coordinate and assist with sample and data collection and data entry for databases and clinical trials being done in the ILD Program.
  • Acts as a liaison between referring physicians and the ILD Program
  • Acts as a liaison between hospital staff, including nurses, physicians, and other departments, pulmonary division fellows, and the ILD Program
  • Supervise the administrative assistant.
