The Opportunity Greenwich House is seeking a compassionate, reliable, and community-focused Outreach Worker to join our team under the CASN SOR-4 grant. This role is rooted in harm reduction and engagement, working directly with individuals who are unhoused or at high risk of substance use–related harm. The Outreach Worker will meet people where they are—both literally and figuratively—building trust, offering resources, and supporting pathways into treatment, recovery, and stabilization. Requirements

  • Experience in outreach, harm reduction, peer support, or community-based human services preferred
  • Strong understanding of substance use disorder, homelessness, and barriers to treatment
  • Ability to engage individuals with respect, empathy, and cultural humility
  • Excellent communication and interpersonal skills
  • Comfort working in the field and engaging hard-to-reach populations
  • Basic computer skills and ability to complete timely documentation

Responsibilities

  • Conduct street-based and community outreach to individuals who are unhoused or at risk of substance use–related harm
  • Utilize harm reduction principles to engage individuals in a non-judgmental, trauma-informed manner
  • Provide education on overdose prevention, safer use practices, and available treatment options
  • Distribute harm reduction supplies and resources in accordance with program guidelines
  • Support individuals in accessing substance use treatment, medical care, mental health services, and social services
  • Build and maintain relationships with community partners, shelters, treatment providers, and referral sources
  • Document outreach encounters, referrals, and outcomes in required data systems in compliance with grant requirements
  • Participate in team meetings, supervision, and required trainings related to CASN SOR-4 deliverables
  • Assist with outreach events and community engagement activities as needed
  • Uphold confidentiality, ethical standards, and Greenwich House policies at all times
