The Outreach Worker will provide supervised family visitation and outreach services to low to high-risk families referred by the DHS. The Outreach Worker provides culturally appropriate, and strengths-based services related to child safety concerns. This position is under the direct supervision of the Program Supervisor.

Requirements

  • High School Diploma, required.
  • Minimum of two years working with parents/caretakers who harmed their children or threatened their children with harm and children who experienced harm or were threatened with harm and experienced trauma and loss.
  • Be at least 18 years of age.
  • Possesses and maintains a valid driver's license.
  • Has daily access to an automobile.
  • Possesses and maintains proof of no-fault insurance.
  • Can obtain an acceptable pre-hire driver's abstract.

Nice To Haves

  • Bachelor's degree in social work, psychology, or a related field preferred.

Responsibilities

  • Behaves in a manner that supports the agency's mission and follows personnel policies and procedures.
  • Provides supervised family visitation and outreach services to all families referred to the program, focusing on parenting, problem solving, communication skills and assisting the client in reducing risk factors and increasing protective factors.
  • In partnership with families, curriculum materials are used to promote positive parent and child interaction and parenting, increase child development knowledge, promote family's interaction with the community and improve health-seeking behaviors.
  • Responsible for transportation, the supervision and monitoring of children during visitations, and home visits.
  • Conducts assessments and observation forms in accordance with funder and state guidelines, and PACT policies and procedures.
  • Accurately completes all necessary documentation, reports and correspondence timely.
  • Regularly consults with Clinical Therapist/Case Manager and/or Program Supervisor.
  • Responsible for supporting the families in completing goals on the Individualized Program Plan (IPP) to reach permanency.
  • Participates in all team and staff meetings, conferences, trainings, and workshops as arranged by the Program Supervisor or funder.
