About The Position

The Outreach Worker/Data Analyst will be responsible for delivering informative workshops in the community on the risks of Substance Abuse as well as ensuring data is efficiently entered into designated systems.

Requirements

  • High School Graduate or GED Equivalent.
  • Previous experience with outreach desired.
  • Maintain current CPR certification from the American Heart Association.
  • Valid Florida Driver's License.
  • Natural Helper or Community Health Worker certification.

  • Bilingual preferred.
  • Good communication and interpersonal skills.
  • Sensitivity in working with diverse cultural groups.

Responsibilities

  • Help in implementing a campaign for the program.
  • Assist with identifying the population and areas most at risk for substance abuse and providing prevention education to those populations.
  • Coordinate and participate in health fairs and other community events to promote prevention activities and provide prevention workshops.
  • Provide education and programming to parents and families.
  • Develop presentation and conduct workshops within the community.
  • Collaborate with existing programs and departments to ensure they are aware of prevention education services.
  • Act as a liaison between CHI and the community.
  • Maintain accurate records of the activities performed and post all activity daily.
  • Complete event logs of all community activities and disseminate the program literature.
  • Assist with follow-up of individuals that need more direct services.
  • Attend scheduled CHI in-services and annual updates. Follow policy and procedure.
  • Maintain regular contact with patients and non-CHI patients to ensure their needs are met.
  • Share data with appropriate staff as necessary. Complete data and submit daily.
  • Collect Data and submit using the data system for prevention by the Behavioral Science Research Institute.
  • Adhere to scope of service of all funders.
  • Maintain personal database of all community contacts and resources on a regular basis accurately.
  • Report any and all incidents to supervisor immediately.
  • Meet monthly goals and submit monthly reports timely.
  • Submit travel by the 10th of each month.
  • Adhere to the Confidentiality Policies and Procedures / HIPAA Regulations.
  • Report to work on time with minimal absenteeism.
  • Perform other duties as assigned.
