Job Summary Reporting to the Program Director of Outreach, Outreach Specialists in the Outreach Program are responsible for engaging with unhoused individuals in New York City, building rapport, and linking them to essential services and support within the community. The role involves conducting street outreach and connecting individuals to the Total Wellness Program, which addresses physical and mental health needs, and various housing supports to facilitate their transition into permanent housing. The Outreach Specialist will work closely with local service providers, drop-in centers, healthcare professionals, and housing authorities to ensure comprehensive care and effective service delivery. This role requires an individual who is compassionate, resourceful, and committed to empowering unhoused individuals to access the services and supports they need to improve their overall well-being and achieve housing stability. The Outreach Specialist will be an active listener, provide high-quality customer service, and respond by exhibiting cross-cultural awareness with the ability to promote and contribute to an equitable and inclusive anti-racist organizational culture and environment.

Requirements

  • High school diploma or GED required. Associate’s degree preferred.
  • Experience working with unhoused individuals or populations with complex needs (mental health, substance use, etc.).
  • Strong knowledge of community resources, housing programs, and social services in NYC.
  • Computer skills and knowledge of Microsoft Office (e.g.Word, Outlook, etc.).
  • Excellent communication, interpersonal, and crisis intervention skills.
  • Ability to work independently and as part of a collaborative team.
  • Commitment tomaintainingconfidentiality and upholding ethical standards in delivery service.
  • A valid driver's license isrequired, with an acceptable driving record thatcomplies withour insurance policy standards.
  • Valid professional licenses and certifications if applicable.
  • May berequiredtoworka flexible schedule including weekends, holidays, and evenings.
  • Ability tosit, stand, walk, bend, stoop, push, pull, lift (up to 25 lbs.).
  • Desire to perform the essential functions of this job.

Responsibilities

  • Conduct homeless outreach and comprehensive assessments of clients’ needs, strengths, and goals through continuous engagement.
  • Engage individuals in supportive conversations toidentifyand assess their immediate needs and provide appropriatereferralsi.e.Intensive Case Management, ACT Team, etc.
  • Maintainaccurateandtimelydocumentation of client activities, interactions, progress, and outcomes in electronic database system[s] (AWARDS).
  • Coordinate and monitor services with internal referrals and third-party providers, including but not limitedto;medical, mental health, housing, employment, and social support.
  • Provide ongoing support to individuals throughout their transition, offering guidance and advocacy as they navigate various service systems.
  • Organizes,maintainsand prepares client charts for intake and discharge.
  • Advocate for clients to have access to necessary resources and services before, during, and after placement.
  • Develop individualized service plans in collaboration with clients and relevant stakeholders once clients accept services.
  • Escorts clients to all appointments, as needed, including but not limited to: 1. Medical, housing, and entitlement appointments. 2. Daily respite sites as mandated byprogram.
  • Assistsin monitoring client medication asrequired.
  • Facilitates client groups as needed.
  • Organizes and distributes clothing donations to clients as mandated byprogram.
  • Completes andsubmitsall necessary documentation and tour reports accurately at the end of each shift as required by Urban Pathways, regulatory and funding agencypoliciesand procedures.
  • Develop andmaintainrelationships with community service providers, housing authorities, and public health professionals tofacilitatea coordinated response.
  • Represent Urban Pathways at community meetings, forums and respond to inquiries as needed.
  • Maintains professional communication both verbally and in writing at all times.
  • Attend staff meetings, training, and supervisory sessions.
  • Responds to all communications including telephone, emailand other oral and written inquiries in a timely manner.
  • Performs all other duties as assigned and as required.
