About The Position

The Outreach Specialist plays a key role in nurturing relationships with internal and external programs in the community, focusing on building enrollment for the CalAIM program. They promote and enroll patients in the CalAIM Program, which encompasses Enhanced Care Management and Community Supports. The Outreach Specialist promotes this program by attending community events, fostering relationships with community partners, leading workshops and presentations to educate the public & clinic staff about the program’s mission, as well, conducting outreach via telehealth or in-person. The Outreach Specialist also provides program support to the day-to-day operations of the program under the leadership of the Program Manager(s) and / or Director of CalAIM.

Requirements

  • High School Diploma (Required)
  • Experience working with vulnerable populations, chronic homeless individuals/families, people with mental illness and substance use disorders
  • Experience with Outreach and Engagement, Patient Care Coordination, Motivational Interviewing and Motivational Interviewing (highly recommended)
  • Community Health Promoter Certification (from Esperanza Community Center) (recommended)
  • Employees are responsible for maintaining individual certifications as required by job function or by law and provide verification and recertification when requested by management.
  • Basic reading, writing, and arithmetic skills required.
  • Extensive software skills in Microsoft Office and telephone protocol.
  • Professional verbal and written communication skills

Responsibilities

  • Engages & outreaches eligible patients telephonically and in-person for the purpose of enrolling patients into the Program(s). 
  • Identifies and works with community and social services already in place for members to assist with enrollment and / or potential outreach opportunities. 
  • Identifies unmet needs and connects members to CBOs that provide community and social services; never leaving referral without a connection/service provided.
  • Conduct information sessions of Program
  • Submit potential patient referrals to the appropriate Managed Care Plans, Program and / or different entity Program is currently contracted with. Revising that all documentation is accurate when submitting. 
  • Ability to work flexible hours, including evenings and weekends.
  • Distribute program information materials to clinics, community and other CBO’s in the area or when needed.
  • Document each outreach contact during or after each outreach excursion in eClinicalWorks, following Outreach workflows for the program(s). 
  • Assist with social support & distribute health promotion materials to the community and / or potential members of program(s). 
  • Connect members to other social services around the area, depending on their current needs and support. 
  • Use motivational interviewing, cultural humility, person centered, and trauma informed care practices
  • Input all patient contact and interactions into database; EClinicalWorks
  • Liaise with staff at partner agencies and locations of outreach to maximize the impact of outreach activities.
  • Possess a working knowledge of all internal SJCH programs. 
  • Complete administrative tasks and duties as requested by Direct Supervisors
  • Organizes and maintains a file system, and files correspondence and other records, as required.
  • Makes copies of correspondence or other printed materials for Program & Team
  • Attend & participate in all multi-disciplinary team meetings and work collaboratively with the program team & clinic staff
  • Provide support to staff/partner agency, help troubleshoot, provide resources/referrals.
  • Prepares and manages all documentation and reporting requirements according to specific site needs including but not limited to Billing/Finance, End of Day reports, referrals, etc.

  • Understand and respond to a diverse population in South LA (homeless, substance users, previously incarcerated, etc.)
  • Pass the Basic Health Worker Examination within six months of employment.
  • Must be able to conduct outreach services within one month of employment.
  • Demonstrate understanding of SJCH policies and procedures.
  • Observes all policies and procedures for use of time-keeping system, including attendance, tardiness, proper clocking procedure, overtime authorization, and that employee nametag is clearly visible.
  • Observe rules and regulations for mandatory requirements including license, fire, disaster, CPR, and annual physical as established by policy.
  • Consistently maintains appearance and dress standards that are appropriate, safe, and in accordance with SJCH standards and policy.
  • Attend meetings as required.
  • Interacts with patients, physicians, staff, vendors, and visitors in ways that demonstrate caring and reflect the SJCH mission and philosophy.
  • Proficient in creating positive telephone communication by answering the telephone by the third ring, identifying self to others when speaking or answering the telephone, listening attentively, asking questions for clarification and offering or seeking assistance to others.
  • Ability to interact with people in a mature, professional, and pleasant manner. Interested in public speaking. 
  • Walking during outreach and standing for long periods of time.
  • Ensures compliance with legal issues including but not limited to patient confidentiality and risk management; ensures compliance with federal, state, and local regulations and FQHC regulations.
  • Ability to manage priorities and workflow in changing environment.
  • Versatility, flexibility, and a willingness to work within constantly changing priorities with enthusiasm.
  • Cash management; supervise all payment plan option, checking donations and cash control established by Finance/Accounting (if applicable for position). 
  • Acute attention to detail.
  • Other duties may be assigned or may be modified as business needs dictate.
