About The Position
Work a full-time 40 hour week, Monday-Friday 9am to 5pm ET and participate in a Saturday coverage rotation You have a passion for working within the community you are a part of, or have been a part of in the past
Requirements
- You have 1-2 years experience with patient navigation, management, or any kind of direct service provision
- You have a Massachusetts Unrestricted Drivers License and Car
- Priority given to Spanish Bilingual candidates.
Responsibilities
- Provide telephonic, written, digital and face to face (in individual's homes) outreach and engagement services for each eligible member.
