About The Position

Join our compassionate and collaborative team as an Outreach Specialist, where you will play a critical role in connecting vulnerable individuals and families to housing resources, treatment services, and essential community supports. This role offers the opportunity to work directly in the community, build meaningful relationships with those in need, collaborate with healthcare and social service providers, and help individuals move toward long-term recovery and stability. The Outreach Specialist works with programs funded through the PATH Grant and Emergency Room Enhancement (ERE) initiatives to engage individuals who are experiencing homelessness or frequently utilizing emergency medical services due to unmet behavioral health needs. Through outreach, case management, and care coordination, this position assists individuals and families in accessing housing, behavioral health treatment, and community-based resources. Requirements

  • Bachelor’s degree in Psychology, Sociology, Social Work, Behavioral Health, or another related human services field from an accredited institution
  • Knowledge of behavioral health, mental health conditions, and substance use disorders
  • Understanding of case management principles and community-based behavioral health services
  • Current driver’s license, acceptable driving record, and current auto insurance required
  • Ability to provide community-based outreach services and transport individuals as needed using a personal vehicle
  • Strong communication skills with the ability to engage individuals in crisis and connect them to supportive services
  • Ability to build effective working relationships with community partners, healthcare providers, and internal teams
  • Ability to multitask, manage a caseload, and respond effectively in crisis situations
  • Successful completion of background screening including criminal record, driving record, abuse/neglect, and fingerprint checks
  • Perform primarily sedentary work including documentation and computer-based tasks
  • Occasionally lift, carry, push, or pull objects weighing up to approximately 10 pounds
  • Perform repetitive hand and finger movements while typing or writing during the work shift
  • Sit for extended periods with occasional walking or standing during community outreach activities

Responsibilities

  • Conduct community outreach to individuals experiencing homelessness or frequent emergency room utilization due to behavioral health needs
  • Build trusting relationships with individuals and families while engaging them in supportive services and treatment opportunities
  • Connect clients to housing resources, behavioral health treatment, substance use services, and other essential community supports
  • Conduct intake screenings and assessments to determine eligibility for services and appropriate care pathways
  • Provide short-term case management and crisis support while assisting individuals in stabilizing and connecting to long-term services
  • Assist clients with accessing community resources such as Medicaid, food assistance, emergency shelter, medications, and support groups
  • Collaborate with hospitals, community mental health providers, and substance use treatment agencies to coordinate care
  • Conduct outreach in community locations including shelters, hospitals, and street outreach environments
  • Maintain and manage a caseload of PATH participants and assist with preparation for enrollment in additional community support programs
  • Document all services provided and maintain accurate records in accordance with program and regulatory requirements
  • Utilize HMIS systems to enroll participants, update referrals, and track program activities
  • Participate in team meetings, staff development activities, and ongoing training opportunities
  • Respond to crisis situations and assist individuals in accessing immediate support services when necessary
  • Build partnerships with community organizations and participate in local meetings addressing homelessness and community support initiatives
  • Perform additional duties as assigned by leadership
