About The Position

The Outreach Specialist at Wayne Metropolitan Community Action Agency is responsible for conducting street outreach to individuals and families experiencing homelessness. This role involves engaging with homeless persons in various locations, assessing their needs, and connecting them to appropriate services and housing options. The specialist will work closely with community partners to facilitate access to resources and support for those in need, particularly focusing on individuals with severe mental illness or substance use disorders.

Requirements

  • Bachelor's degree in human services preferred or a combination of education and experience.
  • Minimum of three years experience conducting street outreach to hard sleepers and those with severe mental illness.
  • Knowledge of Wayne County demographics and services available for the homeless.
  • Strong organizational, collaborative, and interpersonal skills.
  • Experience with the Michigan Homeless Management Information System (HMIS) preferred but not required.
  • Ability to communicate effectively both orally and in written form.

Nice To Haves

  • Experience with motivational interviewing and trauma-informed care.
  • Knowledge of the Michigan Department for Health and Human Services (MDHHS) PATH Program.
  • Familiarity with basic internet applications and programs including Google technology.

Responsibilities

  • Conduct street outreach and engagement to homeless persons in various locations.
  • Target homeless households or individuals experiencing Severe Mental Illness (SMI) or co-occurring Substance Use Disorder (SUD).
  • Collaborate with Out-Wayne Continuum of Care (CoC) to house qualifying individuals for the PATH program.
  • Conduct initial housing screenings using the SPDAT and assess acuity and prioritization of needs.
  • Provide light-touch case management to qualifying individuals and families.
  • Seek and secure supportive services based on individual needs, including housing support and access to identification.
  • Link clients to income supports, physical and mental health treatment, and develop individualized service plans.
  • Attend required trainings from SAMHSA, MDHHS, and Wayne Metro.
  • Maintain accurate data and program records for compliance and reporting purposes.
  • Work collaboratively with agency staff to share information about available services for the homeless.
  • Monitor and enhance effective utilization of time and participate in staff meetings.
