About The Position

Responsibilities

• Establish relationships with local social service agencies and clients.
• Apply Block by Block standard concepts and practices to daily operations.
• Provide daily guidance and direction for outreach operations.
• Develop and adjust deployment plans to meet client needs.
• Utilize reporting processes to update stakeholders on program effectiveness.
• Document all interactions in real-time using the SMART System.
• Participate in monthly calls and meetings to review tracking and discuss challenges.
• Conduct daily outreach activities including patrolling the district and providing hospitality.
• Develop personalized service plans for clients and facilitate navigation of services.
• Respond to calls in the service district and deescalate issues as needed.
