About The Position

The Outreach & Navigator Worker is responsible for breast, cervical, and colorectal cancer prevention and early detection education, promotion of the Cancer Services Program (CSP), targeted outreach activities, and patient navigation. This position is based at United Memorial Medical Center in Batavia, New York, and operates on a per diem basis, with hours as needed, typically from 8:00 AM to 4:00 PM, potentially extending into evening hours based on programming. Some weekend work may be required.

Requirements

  • Effective July 1, 2025, 1 year of experience working with the public, community education, and working with diverse communities.

Nice To Haves

  • Associate or Bachelor's degree in health education or related field.
  • Familiar with medical terminology and computer competence.
  • Community outreach experience preferred.
  • Previous experience in public speaking.
  • Must have ability to read, write and speak English.

Responsibilities

  • Provides breast, cervical, and colorectal cancer prevention and early detection education to the lay community, targeting CSP priority populations.
  • Participates in a minimum of 6 events per quarter.
  • Serves as liaison between CSP, CSP-eligible clients, and partner organizations.
  • Garners at least one earned media per quarter.
  • Makes contact with key stakeholders or government decision makers in their area on a quarterly basis.
  • Works with one provider or healthcare system for patient navigation.
  • Obtains baseline data and works with providers to develop and implement strategies to improve screening rates of the priority population per the guidelines of the CSP grant.
  • Attends CSP staff meetings and local and state meetings as scheduled.
  • Completes monthly reports documenting outreach activities, earned media, contacts with key stakeholders, and patient navigation progress.
  • Provides age and culturally appropriate assessment, interpretation of clinical and laboratory data, and develops and implements age-appropriate interventions and plans of care including education.
