The position involves providing comprehensive case management services to identified chronic homeless individuals. The responsibilities include managing all aspects of an assigned case load, meeting clients regularly both in the field and in the office, and ensuring that all clinical files and reports are complete and up to date. The role requires intensive work with clients as they progress through the program continuum, conducting comprehensive assessments to identify barriers to housing, assisting clients in overcoming these barriers, and securing entitlements. Additionally, the position involves developing individualized housing plans, building a network of support services for clients, preparing clients for housing interviews, arranging housing appointments, and participating in case conferences as necessary. The role also requires liaising with clinical staff in other BRC or outside programs regarding common clients. Fire Safety and First Aid/CPR certification or the ability to pass the test to acquire these credentials is required, along with related duties as assigned.
