This position is responsible for conducting outreach and education on facilitating appropriate use of the Center’s resources, providing enrollment assistance for eligible individuals interested in applying for Public Benefits Entitlements. This position also is responsible for connecting high-risk individuals and facilitate access to community service providers (CSPs) that can meet their health related social needs. This position actively participates in special activities of the Department. It is expected to work within the program directives of the funding source, within the philosophical framework set by the Board of Directors, and within the existing network of community services providers and natural helpers.
