About The Position

The Outreach Education Worker/Navigator is responsible for conducting outreach and education to facilitate the appropriate use of the Center's resources. This role includes providing enrollment assistance for eligible individuals interested in applying for Public Benefits Entitlements and connecting high-risk Medicare and Medicaid beneficiaries to community service providers that can address their health-related social needs. The position requires active participation in special activities of the Department and adherence to program directives and the philosophical framework set by the Board of Directors.

Requirements

• High School Diploma or equivalent required.
• Community Health Worker Certification or college degree preferred.
• Three years of experience in a health care setting preferred.
• Experience and competency in Public Benefit Applications and community outreach work preferred.
• First Aid/CPR Certified.
• A current Hawaii State Driver's License and daily access to an automobile with current registration, safety check, and minimum liability insurance required.
