The Outreach Coordinator at Vita Nova plays a crucial role in supporting homeless youth aged 18-25 by providing outreach services and connecting them to essential resources. This position focuses on building trust-based relationships with young adults experiencing homelessness, utilizing a housing-first approach and trauma-informed practices to facilitate access to services and promote housing stability.

  • Make connections with young adults experiencing homelessness, build trust, and provide access to services to help end homelessness.
  • Provide community-based outreach services in areas where youth experiencing homelessness are known to congregate, including local universities, libraries, streets, parks, and encampments.
  • Act as the advocate and liaison between the homeless community and city/county agencies.
  • Remain calm in crisis situations, think critically, and promote solutions in a volatile street environment.
  • Deliver services with cultural competency and establish effective professional relationships with diverse staff and clients.
  • Work effectively in a team environment.
  • Advocate and connect clients to needed services such as medical, mental health, and substance abuse services.
  • Assist clients with applying for identification documents and other mainstream benefits.
  • Provide diversion and assessment services in the field.
  • Build rapport and utilize motivational interviewing strategies to support vulnerable individuals in accepting housing offers.
  • Enter information and data into the Homeless Management Information System (HMIS) while maintaining client confidentiality.
  • Participate in agency events such as the annual Homeless Point-in-Time Count, case conferencing, trainings, and community meetings.
  • Network with other outreach workers, local shelters, and community organizations to maintain effective working relationships.
  • Be self-motivated and take initiative on behalf of the agency and clients.
  • Maintain records of individuals served, services provided, outreach activities conducted, and other reporting as assigned.
  • Degree in Social Work, Youth Development, or other Human Services related field.
  • Two years experience working with at-risk youth, preferably in a human services/housing non-profit organization.
  • Computer skills, including Microsoft Office Suite.
  • Ability to complete tasks accurately and timely with minimal supervision.
  • Valid Florida driver's license with reliable transportation and ability to be added to the agency's auto insurance policy.
  • Successful completion of a Level 2 Background Check and Drug Screen.
  • Working knowledge of federal/state/local requirements of HMIS Data Standards preferred.
  • Experience/knowledge with federal/state/local housing programs preferred.
  • Experience working in a non-profit organization focused on youth services.
  • Knowledge of local community resources for homeless youth.
