At Community Counseling of Bristol County (CCBC), we are dedicated to transforming mental health treatment in Southeastern Massachusetts. Our Community Support Program (CSP) is currently seeking full-time Community Outreach Case Workers to join our dynamic teams in Bristol and Plymouth counties. This role is pivotal in providing short-term, intensive case management support services to clients who have a history of behavioral health and/or substance use disorders. As a CSP worker, you will play a crucial role in assisting clients to achieve their short-term goals, enabling them to live independently and successfully within the community. By connecting clients to essential community resources, you will help them enhance their quality of life and minimize the need for 24-hour levels of care. In this position, you will establish and maintain meaningful relationships with clients, offering support through outreach in various settings, including the community, clients' homes, inpatient treatment facilities, or other designated locations. You will collaborate closely with both internal and external team members, as well as natural supports, to ensure comprehensive care. Your responsibilities will include conducting thorough Needs Assessments and creating Individualized Action Plans (IAP) tailored to each client's unique needs. You will also connect clients to necessary benefits, housing, healthcare, and other resources that support their goals, providing transportation to essential medical and behavioral health appointments as needed. Fostering empowerment, recovery, and wellness is at the heart of this role, which includes linking clients to recovery-oriented, peer support, and self-help services. You will be responsible for documenting all encounters and maintaining accurate client electronic medical records. Additionally, obtaining insurance authorizations and checking eligibility will be part of your daily tasks. Active participation in staff meetings, in-service training, and other agency-sponsored events is also expected, contributing to a collaborative and supportive work environment.
