About The Position

Outpatient RN & LPN Opportunities - Full-time, Part-time, Per-diem Options Available Across the Capital Region. As a valued member of our outpatient care team, you will provide compassionate, person-centered care to patients in a clinical setting. Whether you're a Licensed Practical Nurse (LPN) or a Registered Nurse (RN), your role is essential in supporting patient wellness and contributing to a collaborative care environment.

Requirements

• For LPNs: Current NYS license to practice as a Practical Nurse
• Graduate of an accredited school of Practical Nursing
• High School diploma or GED
• Ability to lift 20 lbs
• For RNs: Current unencumbered NYS RN license
• Associate's or Bachelor's degree in Nursing preferred
• High School diploma or GED
• Basic Life Support certification
• Minimum 6 months of RN experience
• Ability to lift 20 lbs

Responsibilities

• Escorting patients to exam rooms and obtaining accurate vital signs
• Documenting patient information and updating medication lists in the EMR
• Administering medications safely and transcribing physician orders accurately
• Assisting with treatments, procedures, and patient education
• Performing pre-visit planning and reviewing quality metrics
• Completing referrals and tracking patient compliance
• Monitoring and responding to provider-assigned tasks
• Ensuring proper labeling and handling of patient specimens
• Maintaining a clean and safe work environment
• Participating in daily patient care huddles and team collaboration
• Reinforcing patient and family teaching and providing clinical visit summaries
