About The Position

This position is an ambulatory care registered nurse who will work focusing on our Trinity Health medical group patients. This position will focus on patients with multiple chronic diseases, CHF, COPD. This position will focus on disease education, identifying and mitigating social influencers of health, coordinating needed services and collaborating with the provider offices to ensure integrated care through the healthcare continuum and improved health outcomes of our patients.

Requirements

  • Associates Degree required, BSN preferred.
  • Previous experience in care management in disease management, home health care, hospital, or physician office setting.
  • 5 years of RN experience.
  • Prior electronic medical record (EMR) experience required.
  • Previous project leadership experience required.
  • Strong community resource knowledge.
  • General knowledge of insurance plans and clinical resource management.
  • Excellent oral and written communication skills.
  • Excellent customer service skills.
  • Ability to work independently and set priorities.
  • Proven leadership skills and ability to work effectively in a team environment.
  • Highly organized and detail-oriented.
  • Ability to perform multiple tasks.

Nice To Haves

  • Certification in Case Management (CCM) preferred.
  • Effective behavioral and educational strategies, including motivational interviewing and teach-back method.
  • Recognize the role of cultural, social, economic, and behavioral factors in public health services.
  • Incorporate strategies for interacting with persons from diverse backgrounds.

Benefits

  • Pay range: $69,992-$98,072 based on experience, skills, and education.
  • Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
