About The Position

GENERAL PURPOSE: To ensure the highest degree of quality care is maintained while providing therapy services to the patients and supervising the therapy assistant(s) in the performance of therapy services, in accordance with current federal, state, local standards, guidelines and regulations.

Requirements

  • Possesses a degree from a baccalaureate or Master’s program in physical therapy accredited by the American Physical Therapy Association.
  • Hold a valid and current license to practice physical therapy in state employed.
  • Two (2) years of clinical experience as a physical therapist.
  • Demonstrates good verbal and written communication, and organization skills.
  • Possess and maintains current CPR Certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Must be capable of performing the job functions of this position with or without accommodation.

  • Outpatient/home health experience preferred.

Responsibilities

  • Provides outpatient physical therapy services in accordance with a physician’s written plan of care, focusing on restoring function, reducing pain, and improving mobility for patients across the lifespan.
  • Performs comprehensive evaluations of patients’ therapeutic, rehabilitative, and functional status and collaborates in developing individualized treatment plans tailored to outpatient needs and goals.
  • Assesses muscle strength, joint mobility, gait, balance, posture, range of motion (ROM), and functional movement patterns using evidence-based outpatient assessment tools.
  • Plans, directs, and delivers physical therapy treatments including exercise prescription, therapeutic activities, manual therapy techniques, neuromuscular reeducation, and modalities appropriate to an outpatient setting.
  • Educates and instructs patients and caregivers on home exercise programs, injury prevention, body mechanics, ergonomics, and proper use of assistive or therapeutic devices commonly used in outpatient rehabilitation.
  • Prioritizes patient needs and adjusts treatment strategies based on clinical findings, progress toward goals, and patient-specific functional demands.
  • Communicates with referring providers regarding patient progress, response to treatment, changes in condition, or barriers to achieving functional outcomes.
  • Initiates and progresses treatment plans and, when appropriate, coordinates with support staff or caregivers to ensure continuity and reinforcement of the therapy plan.
  • Completes clear and timely documentation of initial evaluations, daily treatment notes, progress reports, and discharge summaries in accordance with clinic standards and regulatory requirements.
  • Participates actively in discharge planning by determining when goals are met or when further care or specialty referral is appropriate.
  • Maintains open communication and collaboration with clinic staff, front office personnel, and interdisciplinary team members to ensure efficient clinic flow and high-quality patient care.
  • Attends staff meetings, in-services, continuing education sessions, and case discussions to support ongoing professional development and evidence-based practice.
  • Identifies additional patient needs such as referral to specialty providers (orthopedics, neurology, pain management), imaging, or durable medical equipment when appropriate.
  • Provides guidance and clinical supervision to physical therapist assistants (PTAs), technicians, and students, ensuring adherence to state practice regulations and clinic policies.
  • Adhere to all of the Company’s policies and procedures.
  • Perform other duties as assigned.
