About The Position

Pacific Coast Post Acute is expanding its clinical reach, and we are seeking a skilled Outpatient Physical Therapist to lead our growing outpatient program. This is a unique opportunity to provide high-level, one-on-one orthopedic and neurological rehabilitation within a premier post-acute setting. You will work with community members and recently discharged residents, helping them bridge the gap between facility care and full functional independence. Why Lead Our Outpatient Program? Requirements

  • Current, valid Physical Therapist (PT) License in the state of California.

Nice To Haves

  • Experience in an outpatient orthopedic or sports medicine setting is preferred.
  • New Graduates with strong outpatient clinical rotations are encouraged to apply!

Responsibilities

  • Outpatient Evaluations: Perform comprehensive initial assessments for community-based patients, identifying biomechanical deficits and functional limitations.
  • Evidence-Based Treatment: Design and implement specialized outpatient care plans, incorporating manual therapy, therapeutic exercise, and functional training.
  • Program Development: Collaborate with the Director of Rehab to grow the outpatient referral network and enhance the program’s community presence.
  • Continuum of Care: Manage the transition of residents from inpatient sub-acute care to the outpatient setting, ensuring a seamless recovery journey.
  • Documentation: Maintain precise, compliant documentation for outpatient billing and progress tracking.
