About The Position
Brooks Rehabilitation is seeking a skilled and compassionate Physical Therapist to join our Outpatient team. This role is ideal for professionals who are dedicated to providing high-quality patient care in a dynamic, patient-centered environment. Location: 14286 Beach Blvd APT 34, Jacksonville, FL 32250 Department : Outpatient - San Pablo Hours: PRN; ~10hrs/week
Requirements
- Current PT license in state of Florida
- Current hands-on CPR/BLS Certification
- Knowledge of federal and state professional requirements
Responsibilities
- Complete the new patient evaluation, establish customized treatment plans and patient goals, and participate in the implementation of patient care programs
- Deliver high quality patient care for multiple rehabilitation diagnoses
- Provide a positive patient experience through patient engagement and progressive treatment
- Effectively communicate both verbally and written
- Promote evidence-based patient care
