Requirements

Responsibilities

  • Assists in development and implementation of Case Management. Provides expertise in area of social work as it pertains to management of patient.
  • Assists in coordination of client care through formulation of appropriate plan of care via accurate assessment of patient's social, financial and insurance situation.
  • Develops short- and long-term plans to facilitate patient care and discharge.
  • Utilizes crisis intervention and problem-solving skills to facilitate and enhance patient and family's ability to meet acute and ongoing medical and psychosocial needs.
  • Networks with community agencies to assure appropriate client access.
  • Maintains documentation and meets department and University Hospital documentation standards.
  • Identifies cases with ethical and legal considerations and makes referrals. Confers with supervisor.
  • Communicates with other health care providers, patients, families, other hospital personnel, and visitors.
  • Demonstrates effective communication with adolescents, adults, and older adults.
  • Maintains system for tracking placement of patients after discharge on monthly basis.
  • Has knowledge of Department and Unit Performance Improvement program and is aware of expected participation. Identifies need for professional growth and seeks development opportunities.
  • Provides education to staff, patients, and community resource affiliates.
  • Performs other duties as assigned.
  • With leader approval, this position is eligible to participate in Social Work Clinical Ladder, an “opt in” program that allows for additional compensation upon two years completion of approved activities.
