About The Position

Aramark Healthcare+ is seeking a Full-Time Outpatient Dietitian at Boulder Community Health in Boulder, CO. The Outpatient Dietitian provides outpatient medical nutrition therapy in a primary care setting to teens, adults and geriatric population. Provides chronic disease management and prevention for diagnoses including but not limited to obesity, overweight, cardiac disease, diabetes, renal disease, gastro-intestinal disease, and malnutrition. Uses technology tools, approved nutrition resources, and patient centered evidenced based guidelines to support patient care and goals. Works collaboratively with Obesity Medicine Specialist, Primary Care Providers, and other related Medical Providers. Plans and administer activities related to patient nutrition care, complying with the standards established by Aramark, regulatory agencies, and clients.Compensation DataCOMPENSATION: The Salaried rate for this position is $65,000.00 to $80,000.00. If both numbers are the same, that is the amount that Aramark expects to offer. This is Aramark’s good faith and reasonable estimate of the compensation for this position as of the time of posting. Requirements

  • Requires a bachelor’s degree or master's degree in nutrition, dietetics or similar field.
  • Registered Dietitian credential with CDR required
  • Requires at least 1 year of relevant experience
  • Earn and maintain applicable certifications, registrations, licenses, credentials and continuing education requirements by the appropriate accredited local agencies
  • Formal eligibility to practice dietetics in geographic location where applicable
  • Fluency in electronic medical record, nutrition care process language, motivational interviewing, patient centered care skill set, weight management training

Responsibilities

  • Manages the development and implementation plan for nutrition intervention in accordance with the Nutrition Care Process.
  • Assesses educational needs and the presence of barriers to learning.
  • Provides nutrition counseling for individuals and groups, taking into consideration any adaptations to teaching methods necessary to meet patient learning needs.
  • Utilizes assessment and evaluation techniques that consider the varied needs of age-specific populations as well as cultural, religious, and ethnic concerns
  • Evaluates achievement of learning objectives by the patient and/or family.
  • Provide appropriate follow-ups in accordance with the patient’s treatment goals, referring patients for interventions as appropriate including but not limited to food access resources, mental health providers and or activity community resources.
  • Provides appropriate and timely documentation that summarizes the nutrition care plan in a patient’s medical record, including nutrition assessment, diagnosis, plan, implementation, and progress towards goals.
  • Maintains clinical productivity standards and accurate record keeping of daily clinical activities, including information for scheduling and billing
  • Demonstrates accountability for the proper use of patients’ protected health information
  • Adheres to established systems and training programs to provide a safe working environment. Complies with all OSHA regulations and other local, state and federal government regulations.
  • Maintains compliance with ARAMARK’s standards of operation. Adheres to ARAMARK’s Business Conduct Policy at all times. Maintains all records and reports to ensure compliance, with all local, state and federal regulations and codes.
  • Maintains compliance with all requirements of federal, state and local regulations and guidelines including the Civil Rights Act of 1964, as amended, the Age Discrimination in Employment Act of 1967, as amended, and the Americans with Disabilities Act, as amended.
