Visit us online at: https://forestviewhospital.com/ The Outpatient Clinician provides psychiatric case-management and therapeutic services to assigned patients with diverse needs to ensure there is a coordination of care between the hospital, community, treatment, therapeutic services, education, and discharge planning. Requirements

  • Master's degree in a mental health discipline and licensed by the State of Michigan required (LMSW, LLMSW, LPC, LLPC, LLP, TLLP)
  • A desire to work in an outpatient setting with both adolescents and adults

Responsibilities

  • Upon the completion of a thorough psychosocial assessment, the PHP Therapist works in a collaborative manner with assigned patients in the development of individualized treatment goals.
  • Oversees the person-centered treatment planning process and will ensure there is a coordination of treatment interventions with all disciplines, updating treatment plans as needed.
  • Facilitates individual and/or family sessions as needed.
  • Conducts group therapy and other groups therapies as assigned
  • Documents each interaction with patient(s) in accordance within accepted clinical standards.
  • Attends all Multidisciplinary Team Meetings, supervision, and staff meetings.
