As a Clinical Dietitian in the Family Center , you'll be an essential part of a mission-driven team, applying evidence-based Medical Nutrition Therapy (MNT) to promote wellness, manage disease, and restore health. What You'll Do: As an Outpatient Clinical Dietitian, you'll be a vital member of the interdisciplinary care team, providing expert nutrition services that promote wellness and improve outcomes. Your responsibilities will include: Conducting comprehensive nutrition assessments. Developing, implementing, and monitoring individualized nutrition care plans. Providing nutrition education and counseling for patients in both individual and group settings. Collaborating with physicians, nurses, and other healthcare professionals to coordinate Medical Nutrition Therapy . Evaluating the effectiveness of interventions and ensuring continuity of care through clear documentation and hand-offs. Educating patients and families with meaningful, science-based nutrition guidance. Serving as a resource and educator for internal teams and external partners. Assisting in Family Center Food Pantry and other hospital initiatives surrounding food insecurity. Supporting initiatives in patient satisfaction, quality improvement, performance excellence, and regulatory compliance .
