Shift: First Shift (United States of America) Outpatient Blood and Bone Marrow Transplant Case Manager Multiple Myeloma Shift Details: Full Time Monday - Friday 8am - 5pm Option to work from home one day per week after orientation Position Overview: We are seeking a highly skilled and dedicated Nurse Case Manager to support our Blood & Marrow Transplant (BMT) and CAR T‑Cell Therapy program. This role is integral to ensure the delivery of safe, coordinated, and patient‑centered care for individuals undergoing complex cellular therapies. Key Responsibilities: The Nurse Case Manager will: Coordinate and manage individualized plans of care for patients receiving blood transplants or CAR T‑cell therapy Collaborate closely with a multidisciplinary team of nurses, physicians, pharmacists, and advanced‑practice providers Conduct patient triage, participate in clinic visits, and deliver comprehensive education to patients, caregivers, and families Serve as a primary point of contact to support patients throughout their treatment journey Ensure timely communication, documentation, and follow‑through across all phases of care Qualifications and Attributes: The ideal candidate demonstrates: Strong clinical judgment and attention to detail Excellent communication and collaboration skills Reliability, professionalism, and a commitment to high‑quality patient care A willingness to engage in ongoing education and professional development The ability to work independently while contributing effectively to a team‑based environment Impact of the Role: The Nurse Case Manager plays a critical role in supporting patients and families as they navigate highly specialized and emotionally significant treatments.
