The purpose of this position is to serve as part of the ambulatory treatment team providing direct behavioral health services to patients and families. Duties may include consultation, assessment, individual/family/group counseling/psychotherapy, education, prevention, medical exam room-based intervention, referral and service coordination with other community agencies and programs. Assist with program development and evaluation as directed. Perform other clinical, administrative, educational and/or research duties (commensurate with training and ethical scope of practice) as needed and directed.

  • Provide behavioral health services including assessment and evidence-based treatment in an ambulatory care setting.
  • Administer clinical interviews and score standardized assessment tools.
  • Diagnose behavioral health conditions and provide psychotherapy/counseling with supporting documentation.
  • Familiarity with evidence-based interventions such as motivational interviewing, problem-focused therapy, mindfulness, cognitive behavioral therapy, and acceptance and commitment therapy.
  • Assist patients in developing skills for disease self-management, improved coping, distress tolerance, stress reduction, and relaxation.
  • Develop and maintain collaborative relationships with primary care providers, ambulatory practice teams, behavioral health team members, patients, and families.
  • Provide behavioral health education to staff, patients, and families in individual and group settings.
  • Work collaboratively with community mental health providers and community support services.
  • Provide referral, case management, and follow-up services for patients with behavioral health needs.
  • Engage patients, initiate referrals, and address barriers to care.
  • Provide accurate and timely documentation in electronic health records.
  • Assist in capturing program evaluation and fidelity measures.
  • Monitor the behavioral health program and identify areas for improvement.
  • Participate in meetings and quality improvement activities.
  • Provide supervisory support to other team members as needed.
  • Master's Degree in a relevant field.
  • Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Consultant (LPCC).
  • Generalist knowledge of a wide variety of behavioral health conditions and evidence-based treatment modalities.
  • Ability to work in a team-based integrated brief treatment model.
  • Up to 1 year of experience as a Professional Clinical Social Worker or Clinical Consultant.
